Children’s Community Nursing

We provide nursing care for children and young people in your home or local community. We help you manage your child’s health needs so they can stay safe and well. We are available Monday to Friday, 8am to 6pm, and weekends and bank holidays, 8:30am to 1:15pm.

About the team

We are a team of specialist nurses and care staff who support children and young people. We help care for your child at home or at nursery or school, so they can stay well and avoid hospital stays where possible. We can also provide respite care within children’s homes to give families extra support.

Who this service is for

We can usually help if:

  • Your child is under 18
  • Your child has a nursing need
  • Your child is registered with a GP in Surrey or North East Hampshire

We may not be the right service if:

  • Your child is 18 or over
  • Your child does not have a nursing need
  • Your child is not registered with a GP in Surrey or North East Hampshire
  • Your child is registered with a Surrey Downs GP

Questions and answers

How do I get help from this service?
Your child can be referred by a hospital team or your GP. You can also refer your child yourself. We will then contact you and, with your consent, work with the team involved in your child’s care.
What happens at the first visit?
A nurse will visit your home or see your child in the community. They will talk with you about your child’s needs and care. Together, we will plan how to support your child.
Do I need to bring anything?
You do not need to bring anything for a home visit. It can help to have any information about your child’s health or care. You can also share any questions or concerns you have.
Do I need to wait in all day for a visit?
No, we will give you a 2-hour time slot. This helps you plan your day. The nurse will visit during that time.
Will we see the same nurse each time?
You may not see the same nurse at every visit. We have a team of nurses working across Surrey. The assigned nurses for the visit will have access to your child’s notes and will understand your child’s needs.
When might this service not be right for my child?
We may not be the right service if your child does not have complex or long-term care needs. We also cannot help if you live outside our area. If your child needs urgent medical help, contact your GP, NHS 111, or 999.
