Referrals

Please note that between 18:00 on Friday 9 January and 08:00 on Monday 12 January, our online referral site is undergoing essential maintenance.

You will not be able to submit an online referral during this time.

We accept referrals for the services listed below. Please scroll down the page to access the online referrals portal.

Parent/carer and professional referrals

Referrals for continence, occupational therapy, speech and language therapy and physiotherapy can be made directly by parents/carers as well as  the child’s health visitor, GP, therapist, early years’ service, hospital paediatricians, audiology service, Mindworks Surrey, and other agencies.

• Paediatric Speech and Language Therapy (SLT)
  • Children from 0 to 18 years with a communication difficulty relating to a stammer, recently acquired brain injury, voice and/or acute deteriorating illness
  • For children over the age of 30 months (Early Years).
  • Parents/carers of children under the age of 30 months should visit the Speech, language and communication development page.
• Paediatric Speech and Language Therapy (SLT) Dysphagia

For any service not listed above, please contact your GP or Health Visiting team for support and guidance on how to refer.

Professional referrals only

Only the child’s health visitor, GP, therapist, early years’ service, hospital paediatricians, audiology service, Mindworks Surrey, and other agencies can make a referral to the following services

• Education Health Care Needs Assessment (EHCNA)
• Tongue tie (Ankyloglossia)
• Parent-Infant Relationship Service (PaIRS)
• Community Developmental Paediatric Service (DPS)
