We support your health and emotional wellbeing if you are in care. We work with children and young people aged 0 to 19 across Surrey who are looked after by the local authority. We help you stay healthy, feel supported, and get the care you need.
We are the Children in Care Team, supported by Family Health Teams, including Named Nurses, Specialist Nurses, and Senior Staff Nurses for Children in Care. We work with you, your carers, and the local authority to support your health and emotional wellbeing. We help make sure you get the personalised care you need for a healthier future.
Who this service is for
We can usually help if:
You are a child or young person in care
You are looked after by Surrey County Council
You live in Surrey but are looked after by another local authority
You need support with your health or emotional wellbeing while in care
We may not be the right service if:
You are not a child or young person in care
You are not being looked after by a local authority
You do not live in Surrey and are not placed here by another local authority
Getting help from this service
Professional referral only
This service accepts referrals from professionals, including GPs, only. If you're unsure who can refer you, contact us for advice.
Questions and answers
How can you support me?
We support you with all areas of your health. We can guide you to other services if needed and help you make informed choices. We work with you, your carers, and professionals to create and review your care plan.
Where will I be seen?
You can be seen in your home, at school, or in the community. We can also meet at a place where you feel comfortable. We will agree this with you. Or if you prefer we can contact you by telephone or video call.
What happens at my first appointment?
You will meet with a nurse to talk about your health and wellbeing. We will ask questions and listen to your views. Together, we will agree what support you need.
How often will I be seen?
You will usually have a health review once a year. You can contact us between reviews if you need support. We are here to help when you need us.
Will my information be kept private?
Yes, your information is kept confidential. We will only share it with your consent. We may share information if there is a risk of serious harm, and we will try to discuss this with you first.
Who will be at my health review?
You will be there, and a nurse will support you. Your carer or key worker may also attend, depending on your age. This helps us give you the right support.
If I am not living in Surrey where will my health review take place?
Normally your health review will take place local to where you live. Your Children in Care team will let you know.
