This page provides the information and documentation needed to transfer a child or young person to Surrey Child Family Health 0-19 Services including health visiting, school nursing and children in care.

About this service

This page is for use by professionals to provide clear and relevant information about the family, children’s development, health assessments, key professionals working with the family, and presenting concerns including safeguarding and future meeting dates.

Who this service is for

We can usually help patients who are:

  • Are a Family moving into the Surrey area
  • Are a child attending a school in Surrey
  • Have a child up to the age of 19 years attending sixth form attached to a Surrey mainstream school
  • Have a child who is Home Schooled

We may not be the right service for patients if:

  • You are a family with no children under 19 years of age
  • You live outside Surrey without a child attending sixth form attached to a Surrey mainstream school
  • Your child is over 19 years of age
