When your child reaches two years of age, you will be offered an Early Years Review if they attend Ofsted registered childcare (e.g. a nursery, pre-school or childminder).
At 27 months of age, your child will be offered a Health Review. You will also receive two Ages and Stages Questionnaires in the post.
You can read more about the Early Years Review, Health Review and Ages and Stages Questionnaires in the sections below.
Please bring your child’s ‘Red book’ to both reviews.
If your child goes to Ofsted registered childcare, their key person will carry out a review as well.
The results of the Early Years Review should be kept in your child’s personal child health record ‘Red book’ and will be reviewed by the health professional during your child’s Health Review.
Read more about the Early Years Review on the Surrey County Council website.

Health Review
Please look at the two-year-old page in your child’s Personal Child Health Record ‘Red book’ which highlights the many new skills they may be developing at this age. The Health Review is an opportunity to discuss your child’s progress including information on:
At two years old, your child will be due their flu vaccination. Babies and pre-school children receive immunisations through their GP surgery. The vaccination schedule for babies and young children is available on the NHS website including a measles page and a MMR (measles, mumps and rubella) vaccine page.
If you have any queries concerning your child’s progress, please contact us through the contact us or attend a child health drop-in.
You will also receive two Ages and Stages Questionnaires in the post and will need to complete both the Ages and Stages Developmental (ASQ-3) and Social and Emotional (ASQ:SE-2) questionnaires no sooner than two weeks before your Health Review appointment to ensure it is a true reflection of child’s progress.
The ASQ: SE-2 is titled 30 months but developed for children from 27 months.
The information you supply will help reveal your child’s strengths, and determine if there are any resources or services available that may be useful to support your child.
If you would like support to complete your child’s ASQ questionnaire, please contact our contact us.
