Before making a referral to Occupational Therapy, it’s important to consider what support has already been tried. The information can support you with this, offering practical strategies to try at home or in school. Gathering relevant information and exploring these approaches can help meet the child’s needs and ensure referrals are made when specialist input is needed.

About this service

This section contains referral forms, guidance and supporting documentation for professionals referring children and young people to Occupational Therapy services.

It provides information on referral criteria, eligibility, required supporting information and the referral process to help ensure referrals are appropriate, complete and directed to the right service. Please review the documentation carefully before submitting a referral.

Referrals can be made by: School or nursery staff, health professionals (GP, health visitor, therapist, paediatrician), early years services, audiology, Mindworks Surrey, and other agencies and parents or carers.

Before making a referral, Ordinarily Available Provision (OAP) must be implemented for at least three months.

This includes: Embedding strategies into everyday activities, making reasonable adjustments in the setting, using advice from the self-help guide. Educational settings should also contact their local Occupational Therapy team to request a link OT to support delivery of OAP.

Referrals must include evidence of difficulties in everyday activities and the support already provided.

This should include at least two of the following documents: OAP checklist, everyday living skills checklist, the plan do review cycle record and the resource pack record sheet included in the training section of the website.

Who this service is for

We can usually help patients who are:

  • A child’s daily activities are being affected
  • Difficulties are impacting home, school or community participation
  • Strategies and support have already been tried
  • Previous interventions have not resolved the difficulties
  • Specialist Occupational Therapy input is required
  • The referral includes relevant background information

We may not be the right service for patients if:

  • No strategies or adjustments have been tried yet
  • Further information is needed before referral
  • Existing advice and resources have not been explored
  • Needs can be met through universal or targeted support
  • The concern does not relate to everyday functional activities
  • Another service is better placed to support the child
